The Collaborative Residents Enabling Assisted Transitional Engagement (CREATE) program is seeking to hire a Community Life Skills Support Workers to join the team. 1-year contract, 6-month contract, and casual/relief positions are available.
Responsibilities
- Participate on a high performance, multi-service team to provide support to mental health clients in group living settings and with integration into the larger community
- Monitor, review, and update client care plans (OCAN, GAIN, Life Skills Assessments, etc.) on a scheduled basis and revise plans as necessary, ensuring flexibility to meet clients’ needs
- Provide one to one and group supports with individual goal setting and skills development related to life management, independent living, self-confidence, personal health and wellness, vocation, recreation, and social engagement, etc.
- Support and assist clients with ADL tasks to maintain and enhance functional abilities
- Provide support services to clients, including scheduling, arranging transportation and/or accompanying clients to appointments as necessary
- Assist clients to maximize abilities, develop skills, and promote community independence
- Establish supportive therapeutic relationship with clients and advocate for and with clients
- Keep residential group living homes in safe, working order, conducting frequent and regular cleaning and general maintenance of the homes in collaboration with clients (i.e., garbage collection, recycling, overall cleanliness of the homes, etc.)
- Maintain ongoing and consistent communication with CMHA Durham’s CREATE Management on internal/external concerns, pest control issues, snow removal, landscaping, etc.
- Report maintenance issues on a weekly basis (or as necessary) to CREATE Management
- Deliver medications to clients according to established schedules and monitor medications for effectiveness and side effects
- Assist clients to maintain good personal hygiene
- Recognize and report changes in clients mental or physical condition
- Work with community resources involved in the care of the clients as directed
- Assist clients to establish peer and family support networks
- Perform routine housekeeping tasks for a safe and comfortable environment
- Receive monthly rent arrears reports from finance department and work with clients and their income support to resolve rental arrears/defaults
- Facilitate and support movement into and out of the residential group living homes and assist clients with transition into independent or supportive arrangements (i.e., long-term care facilities)
- Collaborate with all members of the client care team
- Document client contacts and create daily logs, summaries, and other reports in a timely manner as outlined in the CMHA Durham policies and procedures
- Other duties as assigned
Requirements
- Post-secondary education in a related area of study (i.e., mental health) and a minimum of two years related experience or equivalent combination of education and experience
- Sound knowledge of the Mental Health Act, Health Care Consent Act, Personal Health Information and Protection Act, the Forensic Mental Health System, social service benefits, and related legislation in Ontario
- Sound knowledge of psychotropic medications as well as some knowledge of medical medications
- Ability to develop and maintain a positive therapeutic relationship with clients
- Capacity to work with individuals of diverse backgrounds
- Proficiency developing and maintaining positive relationships with community partners and representing the agency in a professional manner at all times
- Ability to perform tasks that are necessary for the position (i.e., lifting/transferring clients)
- Excellent communication (written and verbal) skills
- Competence organizing and presenting information
- Flexibility to change
- Ability to work independently when necessary
- Critical and creative thinking skills
- Valid Ontario Driver’s license, access to a dependable vehicle in safe working order and appropriate insurance coverage
- Availability to work a variable work schedule, including days, afternoons, midnights and weekends
- Knowledge of MS Office (Word, Excel, PowerPoint, Access and Outlook), and Internet
