Nurse Practitioner-Led Clinics (NPLC) offer an innovative solution to improve access to high-quality health care in Ontario.
Nurse Practitioner-Led Clinics integrate health promotion, disease prevention, chronic disease management and care coordination for clients of all ages and their families. Additionally, CMHA Durham NPLC provides specialized mental health care to diagnose, monitor and treat symptoms of mental illness in a safe and supportive environment.

Nurse Practitioner Led Clinic Hours of Operation:
Monday- Friday: 9:00am-4:00pm
Extended Hours are offered on Tuesday: 3:30pm-6:00pm

The CANDID program is for vulnerably-housed individuals who would benefit from a Primary Palliative Care approach. CANDID aims to facilitate choice and dignity for individuals with a life-limiting diagnosis. An outreach Nurse Practitioner and Registered Nurse will facilitate pain and symptom management in non-traditional settings.
