Housing and Case Management
CMHA Durham Housing and Case Management Program provides people with community-based case management and safe, affordable housing. The program works collaboratively to promote client-centered recovery and growth.
CMHA Durham has a long history of providing supportive housing and case management services in Durham Region. The program provides ongoing client-directed support to maximize their level of community integration. Subsidized independent housing units are provided in collaboration with area landlords for individuals along with case management services. Group living accommodations provide a shared residence for individuals working towards living independently in the community.
Using a multi-service approach, the program team addresses a range of client needs and provides flexible portable services to clients whose needs, intensity of service use and housing location may change over time. Case managers provide the following services for clients:
- Assessing strengths and needs
- Creating a recovery plan
- Creating a crisis plan
- Medication management
- Providing information
- Family/caregiver support
- Life skills instruction
- Assisting with financial management
- Coordination and linkage to other services and supports
- Assistance accessing services or entitlements
- Creating a discharge plan
Based on the integration of our services, Housing and Case Management clients also have access to other services offered by CMHA Durham including Nursing services and Rehabilitation Services.
